By submitting the online registration form below, I agree to allow my child to participate in Vacation Bible School and its affiliated activities at Trinity United Methodist Church. I have received a copy of Trinity United Methodist Church’s Waiver, Release & Indemnification Form. As a condition for my child’s participation in Trinity United Methodist Church’s Vacation Bible School, I, for myself, my child, my executors and assigns, further agree to release and forever discharge Trinity United Methodist Church, Trinity Outreach Program, Spotted Horse Ministries, the Shelby County Fairgrounds, their members, Boards of Trustees/Directors, ministry leaders, agents, officers, employees and volunteers from any claim that I may have myself or that I could bring on my child’s behalf with regard to any damages, demands or actions whatsoever, including those based on negligence, in any manner arising out of this participation. I have read this entire waiver and permission form, fully understand it, and agree to be legally bound by its terms.  The complete waiver form is available for you to read and download or print if you like.
